Pulp stones are calcified deposits that form inside the pulp chamber of a tooth. The pulp is the soft, innermost part of the tooth that contains nerves and blood vessels, which help nourish the tooth and surrounding tissues. When calcium salts build up within this pulp tissue, they harden over time and form pulp stones, also known as denticles.

Because these calcifications develop deep inside the tooth, they are often difficult to detect in the early stages and may grow larger before being noticed.

Pulp stones typically range in size from 0.05 mm to 4 mm. They can occur in both baby teeth and permanent teeth, and may be found in the crown or root portion of the tooth. Some individuals may have just one pulp stone, while others can have multiple stones in a single tooth.

How Do Pulp Stones Develop?

The exact cause of pulp stone formation is not fully understood. However, dentists believe that age-related changes in the pulp play a major role. Several factors may contribute to their development:

  • Degeneration of pulp cells combined with mineral deposits can lead to calcification.
  • With aging, pulp tissue becomes less dense, allowing calcified material to form.
  • Fat deposits around the pulp chamber may gradually calcify.
  • Teeth that have undergone previous treatments, such as fillings or restorations, may be more prone to pulp stone formation.

Types of Pulp Stones

Pulp stones are classified based on size, structure, and location:

1. Based on Size

  • Fine pulp stones
  • Diffuse pulp stones

2. Based on Structure

  • True pulp stones – made of dentin
  • False pulp stones – formed from calcified tissue debris

3. Based on Location

  • Free pulp stones – lie freely within the pulp
  • Attached pulp stones – attached to the pulp chamber wall
  • Embedded pulp stones – fully surrounded by dentin

Regardless of their type, pulp stones can complicate dental procedures and may trigger discomfort.

Effect of Pulp Stones on Root Canal Treatment

Root canal treatment involves cleaning and shaping the root canals using fine instruments called files. When a pulp stone is present, it can block access to the canal, making the procedure more difficult.

In most cases, dentists carefully remove pulp stones using specialized root canal instruments. Sometimes, a chemical solution such as sodium hypochlorite is used to help dissolve the calcified material.

Because pulp stones can significantly alter the anatomy of the pulp chamber and vary in size and shape, a customized treatment approach is essential for successful endodontic care.
